This waiver applies to participation in Elite Fundamentals, LLC camps, clinics, and instructional events conducted during the 2026 calendar year, regardless of the specific location or facility where the event is held, including outdoor fields, indoor facilities, parks, training buildings, schools, or other training venues.

Waiver, Release of Liability, and Assumption of Risk

as parent or legal guardian of

(“Minor”), grant permission for the Minor to participate in Elite Fundamentals, LLC camps, clinics, and instructional activities.

On behalf of myself, the Minor, and our heirs, representatives, executors, administrators, and assigns, I release, waive, discharge, and hold harmless Elite Fundamentals, LLC and its owners, coaches, instructors, employees, volunteers, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses arising from participation in these activities, except those resulting from gross negligence or willful misconduct.

I understand that participation in baseball and athletic training activities involves inherent risks including, but not limited to:

being struck by baseballs, bats, or training equipment
collisions with other participants
running and sliding
throwing and catching activities
conditioning and agility drills
batting practice and pitching drills
fielding practice and base running
use of baseball equipment, training devices, and facilities

I understand that injuries most commonly occur during drills and live practice situations such as batting practice, throwing drills, fielding drills, base running drills, and other skill development activities, and I acknowledge that coaches and instructors cannot eliminate all risks associated with these activities.

I voluntarily assume all such risks and acknowledge that injuries may occur despite reasonable supervision and safety measures.

I understand that instruction, supervision, and safety measures are provided; however, Elite Fundamentals, LLC cannot eliminate all risks associated with athletic participation.

I further certify that the Minor is physically able to participate and that participation is voluntary.

Participants are expected to follow all camp rules and instructions provided by staff and coaches. Failure to follow instructions or safety guidelines may result in dismissal from the camp.

Indemnification

The parent or legal guardian signing this agreement agrees to indemnify and hold harmless Elite Fundamentals, LLC and its owners, coaches, instructors, employees, volunteers, agents, and representatives from any claims, liabilities, damages, or expenses arising from the Minor’s participation in these activities.

Medical Authorization

In the event of illness or injury, I authorize Elite Fundamentals, LLC staff, coaches, or representatives to obtain necessary medical treatment for the Minor.

I agree to release and hold harmless Elite Fundamentals, LLC and its representatives in the exercise of this authority.

I understand that I am responsible for any medical expenses incurred as a result of illness or injury sustained during participation beyond available insurance coverage.

Medical Condition Disclosure

The parent or legal guardian signing this agreement certifies that the Minor does not have any medical condition, injury, or health limitation that would prevent safe participation in athletic activities, except those disclosed in the Medical Information section above.

The parent or guardian acknowledges that Elite Fundamentals, LLC staff and coaches rely on the health information provided by the parent or guardian.

Elite Fundamentals, LLC shall not be responsible for injuries or medical conditions resulting from undisclosed pre-existing conditions.

Photo and Media Release

I grant Elite Fundamentals, LLC permission to photograph or record video of the Minor during participation in camps, clinics, or instructional activities.

I understand that these images or recordings may be used for promotional purposes including websites, social media, advertising, and promotional materials.

No compensation will be provided for the use of such images.

Duration of Agreement

This release and authorization shall remain valid for all Elite Fundamentals, LLC camps, clinics, and instructional events attended by the Minor during the 2026 calendar year unless revoked in writing by the parent or legal guardian.

Governing Law

This agreement shall be governed by and interpreted in accordance with the laws of the state in which the camp, clinic, or instructional event is conducted.

Severability

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
